Highlights
- •We report a patient with adult-onset Alexander disease (AOAD) mimicking MSA-C.
- •Cerebellar ataxia, spasticity, bradykinesia and urinary disturbance were observed.
- •The patient showed striatal dopaminergic deficit on DAT SPECT.
- •We should recognize that AOAD can affect striatal dopaminergic systems.
